1 Online Registration
If you choose to register through the online, please complete your application as soon as possible. Registration will be on a first come first served basis. Early Bird Registration dead
line is January 15, 2010. Final Registration deadline is March 31, 2010. Once you completed your online registration, it will be submitted to your Local association automatically for approval,
please call your local association President or Secretary and confirm your
online registration information.
After you submit your
applications through online , please print registration receipt , along
with your total amount check made payable to " KCCNA mail to KCCNA

REGISTRATION INSTRUCTIONS AND INFORMATION
Only one registration form is required per family.
Family means husband, wife, unmarried dependent children and dependent parents. Guests (people from outside USA
and Canada) are not considered as part of family and therefore require separate registration of $100 per individual and family will be $250
(excluding accommodation, banquet and food).GUESTS need to register through
MANUAL PDF FORM. Do not include guests with your Family Registration.
Early registration is strongly recommended. Registration will be on a first come first served basis. Incomplete forms or completed forms without
the full payment will not be accepted. Early Bird Registration dead line is January 15, 2010. Final Registration deadline is March 31, 2010.
Send your completed registration form through your local Association. Local association President or Secretary must sign

Registration for the 2010 KCCNA convention will be strictly limited to members of Knanaya Catholic community. Those individuals born
of Knanaya Parents, and if married, whose Spouse is also a member of Knanaya Community in accordance with the principle of endogamy upheld by
the Constitution of Knanaya Catholic Congress of North America. Knanaya Jacobite registrants need to get signature from their local Knanaya priest
and the Knanaya Catholic association President if available in their city. The Registration official will verify the registration. They can register
by selecting any of the normal registration package offered. (They will not be able to participate in any competitions)

ACCOMMODATION
If you register as a "Benefactor" and wish to stay in Gaylord, please enter the number of days you would like to stay
(one room) The standard room in Gaylord has an occupancy rate of 4 people per room, if you would like to purchase additional rooms to accommodate
people in your family as stated in this registration, please enter the number of rooms and the days:
Gaylord Texan is requesting Credit card number of all registrants to hold the reservation. The hotel reservation will be handled by the Accommodation committee only. You
cannot reserve the hotel room directly with the convention-discounted rate

| ADDITIONAL BANQUET TICKETS:
Additional banquet tickets may be required and must be purchased to ensure seats for everyone. Banquet tickets are available only for convention participants. Each banquet chair occupant must have a banquet ticket. |
| HOTEL RESERVATION:
The Accommodation Committee is pleased to assist you with your hotel reservations. You are required to present your credit card at the time of hotel check-in to cover any personal room services |

| TRANSPORTATION: Free Shuttle Bus services are provided from DFW and Dallas LoveField Airport |
| PARKING FEE: There will be a $7.00 / day parking fee if you are driving. |

| MEAL TICKETS:Gaylord Texan is one of the
best hotels in the nation. It has all the facilities we need.
Hotel has 6 different restaurants inside and provides both
American and European cuisines. However this has given us an
opportunity to provide Indian food inside the hotel. We have
worked with various Indian restaurants and selected one of them to
offer Indian food. In order to make this process cost effective
for the attendees, we need to provide a meal tickets count to the
restaurant to provide adequate food during the convention.
These meal tickets will not be available for purchase during the
convention, so we request everyone to purchase them as part of the
registration. |
| For your convenience we are glad to
offer a food price package of $90.00 for one adult (age 13 & up)
and $60.00 for one child (age 6 – 12) for entire convention. This
package price is only available thru January 15th 2010 and must
be purchased along with the registration form. |

However, if you do not want to
purchase the food package, you have the opportunity to purchase
individual meal tickets (for example, 2 breakfast, 3 lunches, 2
dinners etc) only thru the registration form, based on the price
below. |

| SIGNATURE: Valid signature is required for acceptance of registration. By signing the form, participants accept the package selected and agree to abide by any applicable local, provincial and federal rules and regulations. Convention committee reserves the right to deny, suspend or terminate the privilege of participants at any time for any violation of such rules. Incase of dispute Convention Steering Committee's decision will be final. |

SIGHTSEEING
Dallas City tour and Sixth Floor Museum,
After the audio tour of the sixth floor museum, guests enjoy a driving tour of the city of Dallas
including the arts district, fountain place and Fair Park. The Dallas City tour is scheduled for Monday after the convention. The
price for this package is $70.00 per person, which includes transportation and lunch.
San Antonio, A two day package for Monday and Tuesday after the convention. Price per
person is $262.00 (includes transportation, lodging and food)
Trip to WinStar (Casino),
Trip to WinStar Casino is scheduled for Monday after the convention. The price for this package
is $100.00 per person, which includes transportation and lunch.
